
Clinical Trainee Registration
New Notice: The Individualized Clinical Training Plan Request Form is new and now to be used by the Clinical Training Programs/Clinical Superivsors to request approval for an individualized training plan. You may access the new form by clicking on the link below this box titled, "Individualized Clinical Training Plan Request Form"
Notice: All trainees must submit a copy of their transcripts with the Trainee Registration Form. Photocopies of transcripts are acceptable. The Trainee Registration Form will be returned if the transcript(s) is/are not included.
Notice: The North Dakota Board of Addiction Counseling Examiners is now requiring that all training consortiums register their trainees and notify the Board of each trainee’s program completion. In the past, the registration and completion notification was the responsibility of the individual trainee. The Board requires each training consortium submit a Clinical Trainee Registration Form for each new trainee that has been accepted, along with a cover letter stating the academic status of the trainee. At the completion of each trainee’s program, the training consortium is to submit a Trainee Completion Form to the Board. This will allow the trainee to begin the licensure process. The trainee must then request a licensure packet from the Board office or download the licensure packet from the Board website.
